Have the balls to say it in her face and be clear.
The worst thing you can do is not be clear that is over and the reasons, loose ends are very bad and will leave a person thinking of what went wrong for a lot of time.

Just gotta be direct and rip the band-aid off. Be gentle but don't let them think it's possible you can start again or anything.
