game diffifcultly mechanic

So typically at the beginning of many games, theres a set of "easy medium hard" options to determine the difficultly of your play experience. However, what that actually means is up to the particular game dev. What's been your favorite way the difficult was increased? NPCs with greater health pools and/or attack damage? Yourself with a smaller health pool and/or attack damage? Limitations on sort of quality of life features (likes saves, bonus lives, etc). Or maybe rarity and expensive of upgrades? Something else entirely?

I have always been a fan of the hardcore option on games like Diablo, where if you die you lose everything and have to start over. Good way to really feel like what you do and what you get matters and adds some urgency to what you do.

I prefer higher hp but more rarity items, like most mmos.

Introduction of new mechanics that require the player to adapt their gameplay, whether it be strategy or physical mechanics themselves.

Simply increasing health pool and damage of enemies/decreasing your own feels like kinda lazy game design to me. If it's embellished with other things like better AI like in the Bungie Halos, I find that much more interesting.
